Appointments are required. Please refer to our cancellation policy prior to reserving your appointment time. Scheduling an appointment is your acceptance of this policy.
IN OFFICE APPOINTMENTS
Please arrive 5-10 minutes early to your appointment. Prior arrival to your appointment a digital Intake Form must be submitted. Failure to submit the Intake Form before your arrival or at the office, will result in cancellation of your appointment.
Late arrivals will limit the time of your service, as your appointment will end at the scheduled time to accommodate the next patients appointment. We will do our best to accommodate you; however, in some cases it may be necessary to reschedule your appointment. In such cases, the cancellation policy will apply.
We accept Visa, MasterCard, Discover, American Express, checks and cash. State sales tax will be charged where applicable. Please note that if a check is returned to us, a $35 fee will be charged on top of the amount owed to Last Stop 4 Pain, and we will no longer accept checks from you in the future.
The payment is due on or before the first session of each package. It is understood that the package price is discounted due to you purchasing the services upfront. If for any reason you decide to cancel the package before it ends and ask for a refund, every service that was provided will be counted with the full current price and not at the discounted rate. The remaining balance will be returned to you via mail within 10 days.
Gift cards are not redeemable for cash and cannot be replaced if lost or stolen.
We understand that sometimes things can suddenly pop up, but please provide us with a minimum of 24 hours to cancel or reschedule your appointment. If appropriate notice is not given you will be charged 100% of the scheduled service. If you do not notify us at all, (no-show), the service will be considered rendered and 100% of the scheduled service will be charged. Should you arrive late for a scheduled appointment and time does not allow us to perform some or all your treatment, the full amount of the service will be charged. Appointments made last minute, (less than 24 hours prior to service), are not exempt from this policy.
